The last day for all patient services at Grand Concourse will be Friday, October 14th, and the first day at the Family Health Center at Fordham Plaza will be the next Monday, October 17th. Spanning 25,000 square feet and 45 exam rooms, the Family Health Center at Fordham Plaza is a state-of-the-art facility that enables us to maintain the same standard of care that you are used to, delivering primary care, specialty, laboratory, and pharmaceutical services.
Urgent Care and Radiology will continue to be available at 2532 Grand Concourse until February, after which these services will be available at 2300 Westchester Avenue, Bronx, NY, and 1010 Central Park Avenue, Yonkers, NY. Radiology services will also be available at 3400 Bainbridge Avenue, Bronx. NY.
